To ensure that your first visit goes quickly and smoothly we provide an easy way for you to complete the majority of your paperwork prior to coming. You can access all of the documents you will need online, print them out, and complete them prior to your appointment.
To begin the process, simply click on the links below. Please complete each form in its entirety and bring it with you to your first visit. By completing these forms in advance your check-in process will be expedited upon your arrival.

The history questionnaire along with other quality-of-life questionnaires ask very personal questions and will be kept strictly confidential. This information will be used by the doctor to evaluate how best to help you. They will then be used as a benchmark to compare your assessment of improvement after undergoing recommended treatments. Our phyisicans perform clinical research to ascertain their performance so as to continually improve both objective and subjective outcomes – putting you at the forefront of medicine. Things you should know about your first visit
Be aware that your initial visit may take over an hour. The physician will spend a significant amount of time with you at this visit. In addition, our patient information forms and questionnaires have several pages and may be time consuming to fill out. To speed up this process, we have made them available on this website so that they can be completed ahead of time.
If you have completed the forms and have copies with you, we ask that you arrive 20 minutes before your appointment time. If you plan on filling out the forms upon your arrival to the office or have not completed the forms in their entirety, we ask that you please arrive 45 minutes before your appointment time.
- Please bring your health insurance card with you. You are responsible for your co-payment at the time of your visit. Payment may be made with cash, credit or check.
- Please ensure that your primary care physician has processed a referral authorization and that the authorization is communicated to our office prior to your appointment. If you are self-referred, please contact our office so we can assist you in determining if you need a referral authorization.
- During your evaluation, you will be asked to undress from the waist down. You will be provided a comfortable cloth wrap to wear and your examination will be performed with the highest degree of professionalism. You may wish to dress with this in mind.
- You are welcome to bring a friend or family member with you.
- If you are unable to make it to your scheduled appointment, please contact us at least 24 hours before your appointment to reschedule. Failure to reschedule will result in a $25 fee.
- Please feel free to contact us with any questions. We pride ourselves in being knowledgeable and attentive to your needs.
