What to expect during your first visit

We’re eager to meet you and walk you through our comprehensive range of treatment options. Save time in the office by completing your registration (including clinical history) before your visit.

Before your visit

Fill out your health information online through our Patient Portal. Your information is strictly confidential and used only by the doctor to determine the best treatment for you.

If you have completed the medical history form online, please arrive 20 minutes before your appointment time. If you had difficulty filling out these forms online, our staff are happy to assist you — but please arrive 45 minutes before your appointment time to make sure everything is completed before a doctor sees you.

Make sure to bring your health insurance card. You are responsible for your co-payment at the time of your visit. Payment may be made with a credit card or check.

Check that your primary care physician has processed a referral authorization and that the authorization is cleared through our office prior to your appointment. If you are self-referred, please contact our office so we can assist you in determining if you need a referral authorization.

If you are unable to make it to your scheduled appointment, please contact us at least 24 hours before your appointment to reschedule.

During your visit

In order to provide the best possible care, our physicians like to ask plenty of questions and listen carefully. Please be advised that your initial visit may take more than an hour.

You will be asked to undress from the waist down. Cloth drapes are provided during the examination. A friend or family member is welcome to accompany you.

Our staff is happy to answer any questions. We pride ourselves in being knowledgeable and attentive to your needs.
